Veterinary Consent
Download a consent form here:
All treatments are carried out in accordance with the Veterinarians Surgeons Act 1966 and the Veterinarians Surgeons Exemption Order 1962.
Any treatment (other than for sport dogs or a maintenance) can only be carried out once Veterinary Consent has been given.
When your vet has signed the consent form, they can return it to me via email. Once I am in receipt of the signed consent form I can then arrange an appointment for a consultation.
I am fully insured. Most insurance companies will cover for complimentary therapy if it is recommended / referred by your vet, This will need to be looked into by yourself before treatment begins.
I am a member of:
Institute of Registered Veterinary and Animal Physiotherapists
